He was already having the operation because of a skin growth behind his ear. The only thing that really happened was Teller gave the penny to the surgeon and the surgeon gave it back to Teller. The worst thing about the trick was that Penn was in pain with only a local anesthetic, but he seemed to have gotten over it.

Eh, the only thing I can find about this footage is that it was shot during their "Sin City Spectacular" because he had to have a surgery to remove a portion of skin that was growing in his skull, later this footage was deemed unwatchable because "Penn's pain was too disturbing to watch".... As such we will never get to see it because its only in Penn and Tellers private collection
